Some Cypriot lawyers also call the document a "Registration Certificate of Union Citizen". Both names point to the same form.
The Yellow Slip is not a visa, not a Schengen travel card, and not a work permit. EU citizens already have the right to enter and work in Cyprus under EU treaty law. The MEU1 simply registers that you live here. Cyprus is not part of the Schengen Area, so you still need your passport whenever you travel into the EU mainland.

How to Apply for a Yellow Slip Step by Step
The application is made in person at the District Aliens and Immigration Unit (DAIU) of the Civil Registry and Migration Department in your district. Postal submissions are not accepted. A lawyer can attend with you but cannot stand in for you, since CRMD officers may ask the applicant questions directly.
Confirm you've been in Cyprus longer than three months
, or expect to stay that long. The MEU1 is for residence beyond ninety days.
Book an appointment

Register for the General Healthcare System (GeSY)
through gesy.org.cy if you're employed or self-employed in Cyprus.
Get your Social Insurance Service (SIS) number
from the Ministry of Labour, Welfare and Social Insurance.
Open a Cypriot bank account
if you haven't already. Many banks ask for the MEU1 as proof of residence.
Apply for a Tax Identification Code (TIC)
at the Cyprus Tax Department.
Assess your tax residency
